Anxious and Depressive Trends in patients who were taken the Gastrofibroscopy due to Dyspepsia


Abstract
In order to know the anxious and depressive trends in the patients of functional gastrointestinal disorder(FGID) or Non-ulcer Dyspepsia(NUD) and peptic ulcer(PU), we conducted case-control study with 64 cases who had taken the gastrofibroscopy at Holy Family Hospital from March to May 1990 due to dyspepsia with epigastric discomfort and 74 healthy control.
For assessment of anxiety and depression we used Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) devised by Zung.
Major findings obtained from this study as follows
1. The case group consisted of male 33, female 31 and who diagnosed as functional gastrointestinal disorder (non-ulcer dyspepsia)were male 10, female 15 and the remainder were peptic ulcer. The control group were healthy male 50, female 24.
2. The mean age of case group was 37.0(¡¾8.0) in male, 38.6(¡¾12.9) in female and control group was 38.6( ¡¾7.8) in male, 29.5( ¡¾9.2) in female.
3. The mean anxiety scores by SAS in case and control group were 39.7(¡¾8.1) and 35.4 (¡¾7.2) in male and 46.7(¡¾8.7) and 36.9(¡¾6.1) in female, there were significant difference in both sex(p=0.015 in male, p=0.001 in female). For the comparison between FGID(NUD) group and PU group there was not significant in male but in female, FGID(NUD) group
showed significantly high mean score(50.1 ¡¾8.3) compared with PU group(43.8¡¾7.0) (p=0. 0001). The items of mental disintegration (p=0.048), apprehension(p=0.022), dizziness (p=0.017), dyspnea (p=0.021), insomnia(p=0.045) were significantly frequent in FGID (NUD) group compared with PU group in female.
4. The mean depression scores by SDS in cases and control group were 45.2(¡¾8.0) and 37. 4(¡¾9.8) in male and 51.8(¡¾8.0) and 41.8(¡¾5.1) in female there were significant difference in both sex(p=0.0001 in male, p=0.0001 in female). FGID(NUD) group showed significantly high mean scores (55.0 ¡¾ 8.8) compared with PU group (48.8 ¡¾7.0) in female (p=0.036) but in male there was not significant. The items of weight loss(p=0.042), hopelessness(p=0.033), dissatisfaction (p=0.049) were significantly frequent in FGID(NUD) group compared with PU group.
In this study the anxious and depressive trends in patients with dyspepsia were high compared with healthy control which suggested the anxiety and depression did important role as the risk factors of FGID(NUD) or PU.
